What is the forum about?

The Statewide Initiative for Diabetes Management has been developed through consultation across patients, clinicians, LHDs, PHN, ACCHOs and NGOs. Its aim is to support a more coordinated approach for diabetes management to keep people well and reduce the diabetes burden on health services.

The aim of this forum is for all regional diabetes care providers and services to map current diabetes care practices in the community and identify challenges and barriers within our current system.

The forum is an opportunity to review the existing care arrangements for people with diabetes and to establish a more integrated approach to care across Illawarra Shoalhaven.

Who should attend?

Representatives from ISLHD, COORDINARE, ACCHOs, Waminda, HRFS, GPs, endocrinologists, consumers and clinicians involved in providing care to people with diabetes in the Illawarra and Shoalhaven.
